chore(digest): expand_digest/expand_index 只给 planner 与 main agent，不给 worker

worker 不读 graph_overview、不需要按 digest 列成员；命中折叠成员 id 时用 node_detail
(直接取原节点、非折叠感知)即可还原。故从 WorkerTools 移除这两个工具，只保留在
PlannerTools 与 MainAgentTools。
